Age Limit: Candidate should have not exceeding 37 years for Medical Social Worker & Staff Nurse posts, 18-27 years for Junior Radiographer, Junior Medical Record Technician & Nursing Orderly posts, between 18-27 years for Dresser & Plaster Assistant posts as on 22-12-2012 and for remaining posts candidates should asked to refer notification.

Educational Qualification: Candidate must possess 10+2/ Matriculation/ Degree/ Diploma in relevant fields.

Application Fee: Candidates need to pay Rs.125/- Grade Pay Above 2001/- & up to 4600/- and Rs.75/- for Grade Pay Up to 2000/- for all other categories (No fee for SC/ ST/ PWD/ Departmental Candidates, Female Candidates & Ex-servicemen) Remit the requisite fee along with system generated Challan in Punjab National Bank at any Branch.

Selection Process: Selection will be done on the basis of performance in written test and interview for Sr. No. 15 to 21 posts and only interview for Sr. No. 1 to 14 posts.

How to Apply: Eligible candidates may apply through online from www.esich-gurgaon.org website on or before 22-12-2012, after completion of online apply, take print out of the application form, with duly supported with clear / legible attested copies of the relevant certificates and marks statements (in English or Hindi) along with Bank Challan with super hoscribing on “Application for the post of ___, post code__State__ should be sent by Registered post / Speed post to the Dy. Director (admn.), Employees’ State Insurance Corporation, ESIC Model Hospital Haryana, Sector 9A, Gurgaon -122001, Haryana on or before 29-12-2012.

Online Instructions:
1. Candidate have to visit ESIC website www.esic.nic.in/recruitment.php, and www.esich-gurgaon.org for filling the Online Application Registration.
2. Candidates are advised to read the instruction carefully “Guidelines to Candidates Option” given on ESIC website.
3. Fill the application carefully and submit the online application.
4. Before applying online a candidate will be required to have scanned (digital) image of his/ her photograph and signature.
5. Upload scanned Photograph, Signature and Thumb impression.
6. When the application is successfully submitted, a registration number will be generated by the system and displayed on the screen.
7. Candidates should print / note down the registration number for further reference.
8. Candidate will receive registration confirmation by E-MAIL after submitting the online application.
9. Candidates are advised to print the bank challan from website and submit the fee at any Punjab National Bank Branch and fill necessary details in system generated application form as per given space in bank details column.
10. And send system generated application form with necessary documents to the above mentioned address.
